1. Fill in forms 1 and 4 for existing broadband users and 1,3 and 4 for new ADSL connection
2. Read Form 6
3. Fax the application forms for New ADSL processing directly to 1300 731 531
4. Fax the BFH Incentive claim form directly to your local HIC office or, fax it to 02 6124-6758
